Villas At Sunny Acres is a skilled nursing and long term care facility looking for a full time Outpatient Coordinator ( PT or PTA preferred)
This position reports to the Director of Rehab. If the Outpatient Coordinator is a PTA or COTA, they may not function in any of the clinical capacities typically assigned to the DOR.

Duties and Responsibilities:
- All duties and responsibilities shall be performed as set forth in our established policies and procedures.
- Adheres to and assures compliance with Code of Conduct, facility policies and procedures and all applicable rules, regulations and standards as promulgated by Federal, State, and accrediting agencies or regulating bodies. This includes, but is not limited to, the Department of Health, Centers of Medicare and Medicaid Services, and other applicable regulatory agencies.
- All tasks designated and assigned by the DOR will comply with the scope of practice. A PTA/COTA serving as Outpatient Coordinator cannot be delegated responsibilities inconsistent with the Program Manager responsibilities.
- Implements and monitors the provision of facility Rehab Policy and Procedures
- Adherence to Company required policy and procedures and code of conduct.
- Active State Licensure or Registration where applicable
- COMPLIANCE AND INTEGRITY: Models and reinforces ethical behavior in self and to others in accordance with the Code of Conduct; adheres to organizational policies and guidelines; supports compliance initiatives; maintains confidences; admits mistakes; conducts business with honesty; shows consistency in words and actions; follows through on commitments.
Additional Responsibilities (facility specific if not included above):
- Assure Outpatient paperwork is complete for each case, MD/PA/NP signatures complete and logged, work closely with the BOM to assure proper contractual billing and requirements met for triple check, marketing as assigned, developing marketing with OP marketing dept., program development as needed.
Educational Requirements:
- PT, OT, SLP must be a graduate of an accredited Physical/Occupational/Speech-Language Pathology Program
- PTA or OTA and must be a graduate of an accredited Physical/Occupational Therapy Assistant program
Specific Requirements:
- Must hold current and active State license or registration where applicable; SLP must hold a Certificate of Clinical Competence from ASHA
- Three years’ experience in long-term care or related setting preferred
- Supervisory and/or administrative experience in rehabilitative long-term care preferred
- Knowledge of the Medicare PPS System preferred
- Strong organizational and leadership skills
- Effective interpersonal and interdisciplinary communication skills
