Overview
We are seeking a compassionate and dedicated Visiting Nurse to join our healthcare team. The ideal candidate will have experience working with individuals with developmental disabilities and possess a strong background in primary care. As a Visiting Nurse, you will provide essential medical care and support to patients in their homes, ensuring they receive the highest quality of care tailored to their individual needs.
Responsibilities
- Conduct comprehensive assessments of patients’ health status, including physical, emotional, and social needs.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Provide nursing care that may include medication administration, wound care, and monitoring vital signs.
- Assist with discharge planning from hospitals to ensure smooth transitions for patients returning home.
- Educate patients and their families on health management, including nutrition, family planning, and infant care.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Advocate for patients’ needs and coordinate additional services as necessary.
Requirements
- Valid nursing license in the state of practice.
- Experience working with individuals with developmental disabilities is preferred.
- Strong knowledge of hospital medicine and discharge planning processes.
- Familiarity with primary care practices and patient education techniques.
- Understanding of physiology related to patient care and treatment options.
- Excellent communication skills to effectively interact with patients, families, and healthcare professionals.
- Ability to work independently while managing multiple patient cases effectively.
