Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by assessing their needs, developing personalized service plans, and connecting them with appropriate resources. Responsibilities
- Conduct comprehensive assessments to identify client needs and strengths.
- Develop individualized service plans in collaboration with clients, setting achievable goals.
- Provide ongoing support and guidance through motivational interviewing techniques.
- Monitor client progress and adjust service plans as necessary.
- Facilitate access to community resources, including behavioral health services, child welfare programs, and workforce development opportunities.
- Manage crises effectively, employing crisis management strategies to ensure client safety.
- Supervise and mentor clients as they work towards their goals, providing encouragement and accountability.
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with agency policies.
- Collaborate with multidisciplinary teams to coordinate care and services for clients.
- Stay informed about relevant laws, regulations, and best practices related to child welfare and social work.
Requirements
- Bachelor’s degree in Social Work or a related field; Master’s degree preferred.
- Proven experience in case management or social services is highly desirable.
- Strong knowledge of child welfare systems, behavioral health resources, and Section 8 housing assistance.
- Excellent crisis management skills with the ability to remain calm under pressure.
- Proficient in motivational interviewing techniques to foster client engagement.
- Experience in mentoring or supervising individuals in a supportive capacity is a plus.
- Familiarity with workforce development initiatives is beneficial.
- Strong communication skills, both verbal and written, with an emphasis on empathy and active listening.
- Ability to work independently as well as collaboratively within a team environment.
- Commitment to maintaining confidentiality and ethical standards in all interactions.
Job Types: Full-time, Contract
Pay: $26.46 - $28.96 per hour
