Vinson Hall at Home Health. In your as RN you will develop and safely deliver high quality skilled Home Health nursing services to seniors living in their residence here at Vinson Hall Retirement Community.
Primary functions and Responsibilities:

- Perform / Complete Start of Care (SOC), Re-sumption of Care (ROC) and Discharge OASIS documentation
- Performs detailed assessment resulting in the development of a patient specific Plan of Care
- Provides direct nursing care in a Home Health setting under the guidelines of a physician's order

Communicates with other team members regarding the Plan of Care and works closely with team members for optimal outcomes5.
5. Effectively communicates with referring physicians and provides accurate treatment notes and other necessary documentation in a timely manner, per policy.
6. Prepares for patient discharge planning as soon as possible along with informing and preparing the patient, team members and physician.
7. Other duties as assigned.
