For over 30 years, the Violence Intervention Program (VIP) has been the leader in building transformative programs that raise the standard of care for all victims of child abuse, neglect, and sexual assault in Los Angeles County. Today, VIP functions as a public-private partnership between the publicly funded VIP Children's Medical Village at the LAC USC Medical Center and our non-profit, the VIP Community Mental Health Center, Inc. (VIP-CMHC). Our history of quality mental health care is due in large part to the dedicated and talented employees whose efforts have helped improve the lives of children, transitional age youth, and their families.

POSITION SUMMARY

The Case Manager is responsible for providing community-based case management services, linkage, and rehabilitation to children and youth and administration of outcome measures. The Shared CORE Practice Model guides all services children and youth receive.

ESSENTIAL DUTIES & RESPONSIBILITIES

Reporting to the Clinical Supervisor, the Case Manager for the Grant Based program is responsible for the following :

  • Plans and implements individual and group activities that encourage the client's social and emotional growth
  • Administration of outcome measures
  • Identifies the strengths and needs of the youth and family as well as natural supports that exist within the family structure and the community, linkage to resources
  • Follows up with clients and families regarding linkage, resources and services to ensure that their needs are being met
  • Develops, creates, inputs, and implements a clear targeted case management and individual rehabilitative goals in consultation with treating clinician
  • Completes all required documentation in accordance with regulations (e.g. DMH and grant)
  • Enters progress notes into the electronic record (CareLogic) within 24 business hours
  • Performs other duties as assigned.

    Requirements

    BASIC QUALIFICATIONS

  • Bachelor's degree from an accredited university, preferably in social work or a related field
  • Two (2) years documented experience in a mental health setting
  • Bilingual Spanish / English, must be fluent
  • The ability to relate effectively and professionally with children, supervisor, co-workers and staff
  • Demonstrated awareness of, sensitivity to and competence in dealing with cultural and socioeconomic diversity of client population
  • Ability to exercise good judgment and act calmly in emergency situations;
  • Experience and ability to work in a fast-paced environment with good time management skills;
  • Self-starter, independent case manager / rehab specialist with the ability to adapt to change in a fast working environment
  • Valid California driver's license, driving record and personal vehicle insurance acceptable to VIP's insurance carrier
  • Good written and verbal communication skills
  • Working knowledge of community resources that address the special needs of the client base
  • Strong problem-solving skills
  • ELIGIBILITY REQUIREMENTS
