What are the responsibilities and job description for the Home Health & Hospice Intake Coordinator position at VIP Medical Staffing?
A Hug Away, a healthcare agency located in Katy, TX area, is urgently seeking 2 detail-oriented and organized Intake Coordinators to join our growing team. This role will be responsible for efficiently and accurately managing the intake process for home health and hospice patients.
The ideal candidate will possess strong communication and administrative skills, empathy, and proficiency in Kinnser.
Responsibilities:
- Manage all aspects of the patient intake process for home health and hospice referrals, from initial contact to admission.
- Receive and process new patient referrals from various sources (physicians, hospitals, facilities, families, etc.).
- Verify patient insurance information and obtain necessary authorizations for both home health and hospice services.
- Coordinate with physicians, case managers, and other healthcare professionals to gather required patient documentation, including home health orders and hospice eligibility.
- Accurately enter patient demographic, medical, and insurance information into the EHR system.
- Schedule initial patient assessments/evaluations and communicate appointment details to patients, families, and clinical staff for both service lines.
- Maintain organized and confidential patient files and records.
- Serve as a compassionate and supportive point of contact for new patients and their families.
- Collaborate effectively with the interdisciplinary teams.
- Ensure compliance with all relevant regulations and agency policies for both home health and hospice.
- Assist with other administrative tasks as needed.
Qualifications:
- Previous experience in medical scheduling or health information management is preferred.
- Familiarity with medical billing practices and understanding of insurance verification processes.
- Working knowledge of Kinnser/Wellsky.
- Excellent organizational skills and meticulous attention to detail.
- Strong written and verbal communication skills, with the ability to communicate empathetically and professionally.
- Ability to prioritize tasks and manage time effectively in a fast-paced environment.
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
- Ability to maintain strict confidentiality and handle sensitive information with dignity and respect.
- Excellent interpersonal and customer service skills, demonstrating compassion and understanding.
- Bilingual in Spanish is preferred but not required.
