Speech Therapist (Hospice Care)

Position Overview : The Speech Therapist in hospice care provides specialized care to patients with speech, communication, swallowing, and language disorders as part of an interdisciplinary team. This role focuses on improving the quality of life for patients with terminal illnesses by addressing communication difficulties and assisting with swallowing issues to enhance comfort and safety.

Key Responsibilities :

Assess, diagnose, and develop individualized care plans for patients with communication and swallowing disorders.
Collaborate with the hospice care team, including physicians, nurses, and other therapists, to create holistic care plans.
Provide speech and language therapy to help patients maintain the ability to communicate effectively.
Assist patients with dysphagia (difficulty swallowing) to ensure safety and prevent complications such as aspiration pneumonia.
Educate caregivers and family members on how to assist patients with communication and swallowing needs.
Monitor patient progress and adjust treatment plans as needed.
Document assessments, treatments, and patient progress in accordance with hospice and regulatory guidelines.

Qualifications :

Master's degree in Speech-Language Pathology.

Valid state licensure as a Speech-Language Pathologist.

Experience in hospice, palliative care, or geriatric care preferred.

Excellent communication and interpersonal skills.

Ability to work compassionately with terminally ill patients and their families.

Occupational Therapist (Hospice Care)

Position Overview : The Occupational Therapist in hospice care focuses on providing care that enhances the quality of life for patients with terminal illnesses. This role includes helping patients maintain independence in daily activities, reducing pain, and improving overall comfort by adapting activities to meet their current abilities.

Key Responsibilities :

Evaluate patients' physical, emotional, and cognitive abilities to create personalized care plans.

Assist patients with activities of daily living (ADLs) to enhance their comfort and independence.

Provide therapy that focuses on increasing mobility, pain management, and comfort.

Work closely with the interdisciplinary hospice team to coordinate care and ensure patients' needs are being met holistically.

Educate patients, caregivers, and family members on adaptive techniques and equipment to promote independence.

Assess the safety and accessibility of the patient's home environment, recommending modifications or equipment as needed.

Document care plans, treatments, and patient progress in compliance with hospice policies and state regulations.

Qualifications :

Master's degree in Occupational Therapy.

Current state licensure as an Occupational Therapist.

Qualifications :
