The Clinical Social Worker 3 will complete a psychosocial assessment; develop treatment plans that are congruent with the overall health care plan; provide psychosocial intervention; educate patients, their family members, staff and social work students; facilitate compliance with treatment; work with the community to provide needed services; and coordinate discharge planning activities.

Experience REQUIRED:
CSW Level 3: minimum of three (3) years of clinical social work experience with at least one (1) year in a specialty practice area

Clinical social work experience in an acute care setting preferred

Master of Social Work Degree from a Council on Social Work Education accredited program

Independent action(s) required:
Initiate patient/family intervention based on self-assessment of service need(s).
Initiate community/facility referrals based on identified patient/family needs.
Initiate consultation with facility staff regarding psychosocial patient/family needs.
Contact the appropriate Social Services Department to report suspected abuse/neglect/exploitation cases.
Develop intervention plans and related interventions with patients/families/significant others.
Initiate education activities with and for facility staff.
Contribute to quality improvement activities through self-analysis of services and needs.
Participate in service area meetings, training, and patient care conferences.
Supervisory responsibilities (if applicable):
This position may supervise social work students on a voluntary basis.
Depending on unit assignment may supervise other Masters and Doctoral Degree students from other disciplines (e.g. Psychology).

This part time clinical social worker will join the interdisciplinary team supporting the pediatric patients and their family's receiving treatment through the CHoR pediatric nephrology, transplant, or rheumatology clinical services. This CHoR nephrology treatment team has been ranked in the US. News & World Report 9 times in the last 12 years. With two social workers designated to these clinical teams, they will collaborate and share responsibilities to increase outreach, advocacy, and mental health supports for children awaiting transplant, receiving dialysis, living with chronic kidney disease, or coming to one of the clinics for routine follow-up.
The interdisciplinary team deliver services in various clinical locations across the state including Richmond and Fredericksburg, although this social worker will primarily report to CHoR Pavilion. This clinical social worker will develop competence in and remain abreast to applicable regulations set out by CMS and UNOS in order to help ensure treatment meets compliance standards and maintains best practices.
The expectation is for this team member to be present in person to see patients in clinic with the interdisciplinary team, during normal business hours. No on-call or weekends expected. Business casual attire. The Health System follows Centers for Disease Control and Prevention (CDC) guidelines for hand hygiene and fingernail care.
