Starting Salary : $143,969 / yr for 1.0 FTE (prorated for lower FTE) $15,000 annual leadership premium DOE

Job Summary : The Site Medical Director for School Based Health (SMD) provides clinical leadership for the school-based health centers (SBHCs). The SMD-SBHC will be responsible for the direct supervision of the provider staff including coaching and performance management of clinicians assigned to the SBHCs. The SMD will participate as a member of the SBHC leadership team, assist with recruitment of providers and other medical staff for the site, and assist in leading quality improvement activities. The SMD will collaborate with the SBHC program manager to ensure operations support best clinical practices and the outcomes desired by Oregon Health Authority’s SBHC State program office (SPO). The SMD provides primary care in accordance with their level of training, experience and licensure, and will function as an informational resource to VGMHC patients, professional colleagues, the Center, students and the community. The SBHC program providers and staff support the Farmworker Outreach Program each summer and SBHC schedules are modified to accommodate this collaboration. This position is 0.1 FTE clinical leadership, with the remaining FTE providing patient care.
Essential Duties and Responsibilities :
- Provide direct supervision of providers at the Washington County SBHC sites and provide support to Yamhill County SBHC clinical services.
- Provide clinical leadership and mentoring of clinicians at the sites in the practice of evidence-based, timely, effective, efficient, and equitable medical care and :
- Provide clinical input on care and management of individual patients as may be needed by colleagues and co-workers on a day-to-day basis.
- Review cases for CSOC (Controlled Substances Oversight Committee), if appropriate.
- Review unexpected outcomes, including deaths, as part of peer review and QI.
- Participate as a member of the SBHC Leadership Team to :
- Solve clinic problems (schedules, coverage, etc.).
- Resolve personnel issues.
- Implement improvement and other projects at the clinic level.
- Perform responsibilities delegated by administration.
- Implement plans to meet operational, financial, and patient care quality goals, including monitoring of progress toward quality metrics.
- Plan, attend, and help lead SBHC All Staff, provider, and team meetings.
- Participate in school and district events and outreach as needed and determined by the SBHC leadership team and with approval of Medical Director of Community Programs or Chief Medical Officer.
- Provide clinical input to assist in solving patient issues related to quality of care, behavioral issues, etc.
- Attend SMD meetings and act as conduit for information from medical leadership to providers and clinical staff.
- Assist in coordination of provider scheduling, including working with other site leaders to approve vacation and other time off requests, and ensure timecards are approved and time management of providers is monitored.
- Act as liaison, in coordination with SBHC program manager and the Medical Director of Community Programs, between the SBHCs and other pertinent groups in the community, regarding clinical issues and access.
- Be the clinical liaison to other departments or programs within VG, such as Dental, Integrated Behavioral Health and the farmworker outreach program.
- Assist with developing clinical workflows for Farmworker Outreach Program and participate or coordinate appropriate participation in the farmworker outreach events each summer.
- Support the program supervisor and manager in attending to the day-to-day concerns of providers, support staff and management team that may arise.
- Ensure that procedures are in place to maximize the flow of information through the clinics to increase patient safety and provider continuity.
- Assist in implementation of plans to meet the clinical quality objectives of the Health Center overall and the SBHC program more specifically.
- Ensure that clinical training needs identified are met for provider and support staff.
- Provide performance reviews and feedback to SBHC provider staff annually.
- Have regular 1 : 1 meetings with direct reports, at least quarterly, but more often when needed.
- Work with SBHC leadership to recruit provider and other medical staff for SBHCs as appropriate, scheduling time for interviews with providers, lunches, etc.
- Orient new providers to their site and hold check-ins with them regularly.
- Act as a representative of SBHC Program clinical staff with leadership team, assuring good communication and acting as a liaison between primary care site leaders and SBHC team.
- Provide direct patient care in accordance with training, experience and licensure. The scope of practice will be outpatient family or pediatric (up to age 21) medicine.
- Provide comprehensive and culturally competent health care of the highest quality.
- Provide input on case management for patients as needed, working with support staff.
- Meet patient care access standards.
- Provide timely documentation in the medical record of care provided to patient, including education, medications, referrals, diagnostic testing and follow-up.
- Provide supervision and education of students and residents in the health care professions.
- May be required to provide services at VG locations other than primary site.
- Perform other duties as assigned.
- Handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
HIPAA Requirements :
The Site Medical Director will have access to PHI in the course of carrying out their duties. The Site Medical Director uses the PHI for treatment purposes and quality assurance operations. Applying the minimum necessary standard of HIPAA, the designated records set to which this employee will have access include : the demographic and scheduling information in the practice management system, and the full medical record.
