Job Description:
We are seeking a dedicated and experienced MDS Nurse (RN) to join our Long Term Care team at Virginia Gay Hospital. The MDS Nurse will be responsible for completing accurate assessments and developing care plans for residents in accordance with state and federal regulations. The ideal candidate will have strong clinical skills, attention to detail, and a passion for providing high-quality care to our residents.
Responsibilities:
The Minimum Data Set (MDS) Coordinator is a member of the healthcare team. He/She is responsible for completion of the MDS, CAAs, and the development of care plans. He/She will attend care conferences and serve as a liaison between residents and their families, staff, and other disciplines to ensure quality of care and patient satisfaction. He/She works to promote quality of life and keep residents at their highest level of functioning.
- Complete accurate assessments of residents' physical, emotional, and psychosocial needs
- Develop individualized care plans based on assessment findings
- Ensure care plans are implemented and updated as needed
- Coordinate with interdisciplinary team members to provide comprehensive care to residents
- Ensure compliance with state and federal regulations related to MDS assessments
- Participate in quality improvement initiatives to enhance resident care
Qualifications:
- Active RN license in the state of Iowa
- Minimum of 2 years of experience in long term care or a related setting
- Strong clinical assessment skills
- Knowledge of MDS assessment process and regulations
- Excellent communication and interpersonal skills
- Ability to work effectively in a team environment
