Essential Duties and Qualifications
The Intake Coordinator oversees the referral intake process in Central Intake setting. Performs duties and responsibilities in a manner consistent with corporate values and service excellence.

Essential Duties and Responsibilities:
- Encouraging and supporting effective teamwork within the agency.
- Delegating effectively, expecting and allowing staff to complete assigned tasks with appropriate oversight.
- Planning and implementing appropriate staffing arrangements, consistent with resource availability; Optimizing staff productivity.
- Employing qualified staff; assuring that new employees are appropriately oriented to their duties and responsibilities.
- Conducting timely formal and informal performance evaluations, supported by adequate and appropriate documentation.
- Recognizing and rewarding exemplary performance and providing coaching and counseling to employees whose performance is unacceptable.
- Obtaining referral information via phone/fax from physicians’ offices, hospitals, home care coordinators and insurance case managers.
- Working closely with marketing staff to identify problems/areas of concern as related to referral sources.
- Promoting good relations with referral sources and potential referral sources.
- Maintaining adequate knowledge base of all funding sources/regulation; updates knowledge on all aspects of managed care contracts, insuring appropriate insurance verifications and communication with case managers.
- Participates in the initiation of the patient plan of care.
- Compiling, evaluating, and communicating to Agency complete, appropriate data necessary in provision of quality care.
- Coordinating with home services departments and other appropriate personnel to assure proper and timely initiation of appropriate services.
- Communicating with patient, family, and physician the intent and purpose of home care applicable to each patient situation.
- Acts as a liaison between hospital and home care personnel. Duties may include:
- Providing consistent means and vehicle for appropriate sharing of patient care a service date.
- Identifying new service opportunities and takes initiative to investigate.
- Identifying and reporting potential problems appropriately, and takes corrective action or offers solutions, practicing service orientation in representing Agency to hospitals and departments.
- Participating in educational activities to both areas of representation.
- Participates in Quality Improvement activities.
- Other duties as assigned.
- Participating in special assignments, projects and data gathering assigned by manager in a complete and timely manner
- Initiating and developing systems for more efficient gathering, processing and reporting patient data.
Knowledge, Skills and Abilities:
- Graduate of an approved school of nursing with current state license; LPN preferred. Develop/use multiple resources to resolve problems, good communication skills both written and verbal.
- One year experience in home care required, preferably in management position.
- Employee is required to sit, stand and walk frequently, and to lift up to 50 lbs. Over 50 lbs, occasionally.
- Job requires occasional bending, kneeling, reaching forward and overhead, and lifting floor to waist and waist to overhead.
- Procedures require use of both hands for grasping, pushing/pulling and fine manipulation; driving requires use of at least one foot.
