The Advanced Heart Failure Advanced Practice Provider (APP), in collaboration with and supervision by the Advanced Heart Failure cardiologist at Virginia Heart will provide patient care on both an inpatient and outpatient basis as dictated by clinical needs. The APP will participate in the clinical assessment, planning of care, and treatment of the advanced heart failure patient. In addition, the APP will participate in research and quality assurance activities. The AHF APP may also on occasion see general cardiology patients. The AHF APP will travel the Fairfax and Loudoun offices with the AHF Cardiologist. The APP will improve the continuity of care and promote the practice of Virginia Heart in a manner consistent with Virginia Heart’s mission of providing excellence in cardiovascular care..

We are seeking experienced professionals to join our team in our outpatient cardiology setting. RESPONSIBILITIES:
- Obtains medical history and perform physical examinations.
- Performs clinical assessments and develops treatment plans.
- Initiates, monitors, and adjusts medications in accordance with the APP Practice Agreement.
- Manages patients with complex cardiovascular diseases.
- Orders and evaluates laboratory, diagnostic, and radiographic tests.
- Completes medical records and billing at the end of each day.
- Coordinates care with rounding physician or office-based physician.
- Provides education to patients and their family members.
- Provides pre and post procedural education for patients scheduled for cardiac testing.
- Coordinates care between other physicians, nursing staff, and interdisciplinary providers.
- Makes appropriate referrals to community and other interdisciplinary providers.
- Refers to appropriate Virginia Heart Programs: Heart Failure Program, Research Program, Sleep program, Structural Heart Program.
- Acts as liaison for the physician in communicating with patients and family members.
- Acts as a resource for clinical staff for daily clinical issues.
- Regularly attend APP & Provider Meetings.
- Hospital Specific: Perform daily hospital rounds, coordinate daily work with the rounding MD at the beginning of day, prioritizing patient care based on severity of illness. Maintain communication with the rounding physicians. Communicate unresolved issues to the MD at the end of the day. Coordinate scheduling of inpatient stress test, cardiac cath, and other inpatient and outpatient testing. Provide discharge education and outpatient follow up orders and plan Maintains communication with the rounding physicians.
- · Office Specific: Maintains own schedule of approximately 15 patients in 9-hour day. Physician is available on site for consultation and collaboration as needed. APP does not see new patients.
REQUIREMENTS:
- Graduate of an accredited Nurse Practitioner Program or Physician Associate Program (Master’s level of preparation preferred)
- BLS/CPR and ACLS certified
- NP Credentialed by the ANCC (American Nurses Credentialing Center). PA credentialed by NCCPA (National Commission on Certification of Physician Associates).
- Licensed in State of Virginia
- Experience in Cardiology-preferably as an APP
- Have appropriate hospital privileges
- Able to work collaboratively
- Attend CME annually
