RN Care Management is responsible for care coordination of each client receiving services from Virginia Home Care Partners. A critical component of the RN Care Management role is to demonstrate the company’s culture of compassion, accountability, and results by displaying the highest standards of clinical integrity, superb follow-through and consistency, and exceptional customer service and client satisfaction. RN Care Management works in coordination with the Care Support Manager and Administrator to ensure client care, compliance and Care Team Support is maintained with excellence.
QUALIFICATIONS – KNOWLEDGE REQUIRED:
To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and or ability required.
- Must be a licensed Registered Nurse in the State of Virginia
- Must be a graduate of an accredited school of nursing
- Three years clinical experience, preferably in Care Management, in-home care, home health, or hospice
- Must demonstrate an ability to practice independently the delivery of nursing care
- Must be able to supervise and instruct others
- Ability to read and write in English.
- Reliable vehicle required for use when traveling to worksites
- A valid Virginia State Driver’s License
- Proof of current auto liability insurance
- Computer literate, with experience in different computer programs
- Strong grasp of legal and ethical issues pertaining to confidentiality
- Creative, flexible, and innovative

- Excellent written and verbal communication and presentation skills
- Leadership skills promoting a positive team culture
- Ability to apply problem-solving techniques to the Care Management process

DUTIES, RESPONSIBILITIES, and ACCOUNTABILITIES:
The following duties and responsibilities reflect the essential functions for this job, but do not restrict the tasks that may be assigned. The Director may assign or reassign duties and responsibilities to this job at any time due to reasonable accommodation or for other reasons.
- Reports to the Administrator and/ or Care Support Manager
- Performs the essential activities of Care Management including assessment, planning, implementation, coordination, monitoring, evaluation of outcomes, and general interventions across Care Management practice and processes, and makes appropriate referrals to other disciplines
- Performs initial evaluation and assessment of Client needs
- Establishes the Care Plan and educates Care Management as well as Care Team Partners on delivery of care
- Documents nursing observations, interventions and evaluations of the care being provided in accordance with agency policy and VDH regulations and compliance
- When assigned, supervises Care Team Partners
- Participates in team conferences, as requested, and attends staff in-services
- Leads compliance trainings for Care Team Partners including Personal Care credentialing and competency testing
- Thoroughly explains the Company’s mission and value to the Client and Client’s family members, doctors, providers, etc.
- Explains the RN Care Management role and services of the company, describes Client’s rights, and provides contact information for complaints
- Responsible for accurate, timely, and ongoing evaluation and reporting of client’s needs, services, and outcomes
- Provides supervision, coaching and support to Care Team Partners who are responsible for the implementation of the Care Plan
- Documents assessments, progress, re-evaluations, and other pertinent information in the client’s documentation system
- Performs other duties as required.
Pay: $44.50 - $49.89 per hour
