Chronic Care Coordinator
Large physician office is seeking a full-time Chronic Care Coordinator to conduct telephonic counseling to our Chronic Care Management patients. Responsibilities include documenting and communicating information to and from the providers for effective and coordinated care. CCM coordinator will work closely with our primary care providers to ensure patients receive consistent and comprehensive care through regular outreach, medication monitoring, and care plan updates, all while aiming to improve patient outcomes. CCM coordinator will be tracking patient progress based on individualized care plans for each patient’s specific chronic condition and documenting care activities in the electronic health record.
Job Type: Full-time
Pay: $18.00 - $21.00 per hour
Expected hours: 40 per week
