Responsibilities:- Meets with patients and their families for psychosocial needs assessment and examines social support, appropriate community resources, and coping mechanisms.
- Documents all communication accurately and appropriately.
- Provides psychological support to patients and families. Provides support to clinic patients and their families during hospitalizations.
- Provides follow-up counseling on an ongoing basis.
- Acts as a patient/family advocate in navigating the health care system.
- Develop, facilitate, and maintain support groups to enhance the quality of care provided to patient and their families.
- Provides information to patients and families regarding resources both in local communities and those geographically distant,
- Collaborate with the clinical team to meet both patient and family’s needs through ongoing communication with clinical team.
- Develops and maintains working relationships with community agencies, health care organizations, and other providers to promote a broad base of support for patients and families.
- Maintain knowledge of organizational policies, NASW code of Ethics, and/or best practices when operating within the healthcare setting.
- Demonstrates knowledge of age-specific differences in the patient population that include late adolescence, adulthood, middle age, and the elderly. This includes physiological, psychological, and developmental differences unique to each group.
- Performs case management services ( i.e., resource coordination, developing, coordinating, and monitoring a goal plan) with patient and families
- Master's degree in social work from an accredited university required
- Must be a Licensed Maters Social Worker (LMSW) or be able to obtain licensure within 6 months.
Oncology experience preferred or relevant hospice, palliative or hospital experience at least a 1 year
- Knowledge of Microsoft office products.
- Must be willing to travel between locations as needed.
