Summary:
Responsible for the psychosocial assessment, planning, implementation, monitoring and support of patients through the continuum of care.
Will have a dedicated caseload and will facilitate coordination and collaboration among members in social resource navigation and overall case management.
Utilizes independent critical thinking and collaborates with multidisciplinary team to address patient’s needs and transition into different levels of care when applicable.
Works in accordance with Federal, State and Local Standards, Guidelines and Regulations, and the Organization’s Policies and Procedures.
Position Responsibilities:
Care Coordination and Case Management – Coordinates and implements appropriate care plan with dedicated caseload through psychosocial assessment, multidisciplinary rounds and patient advocacy. Communicates and educates patient, family and healthcare team on the plan of care and makes referrals to appropriate services and agencies. Provide patients with health education and coaching to help manage their condition.
Documentation – Appropriate and complete documentation of psychosocial review and transitional plan in the medical record and in the third-party documentation systems (Medical Social Worker).
Team and Community Involvement – Possess knowledge of internal and external referral network as well as public, private, and internal partners and health agencies available to serve patients. Acts as a liaison and subject matter expert with social service agencies in establishing effective professional relationships and knowledge sharing. Possesses knowledge of state and federal hospital compliance. Identifies and reports compliance issues as appropriate.
Position Qualifications Required / Experience Required:
Required Experience:
1-year Medical Social Work experience required.
3 years' experience as Clinical Social Worker preferred.
Required Education:
Graduate of an approved Social Services Program, minimum education of Bachelor’s Degree in Social Work.
Training / Certification / Licensure:
Current certification or licensure by the State of New Jersey
