Job Summary:
Certified Nurse Consultant for Wound & Ostomy patient referrals for acute care inpatient services. Services provided after consultation generated by physician or nursing departments for individualized patient assessment and treatment recommendations. Adjunctive Nursing services include staff education, interdisciplinary coordination of care, clinical leadership, clinical trials and studies and performance improvement activities.
Position Responsibilities
Certified Nurse Consultant – assessment, diagnoses, planning, implementation and evaluation of wound and/or ostomy patients
Support Staff education- unit based education, system-wide education programs, vendor product education/coordination, competency training for staff, case studies
Interdisciplinary Coordination of Care- Collaborates with multidisciplinary team such as licensed providers, physical therapy, nutrition, nursing services, home care, ambulatory care, case management & social services in the coordination of wound, ostomy and skin care needs of patients.
Clinical Leadership- Representative on system-wide and divisional committees. Nursing leader who serves as change agent and role model for nursing staff and coordinates projects and activities related to wound, ostomy and skin care system wide.
Clinical Trials and Studies- Collaborates with Materials Management/Supply Chain and PVAC committee for wound, ostomy, bed therapy and skin care product trials, clinical studies and evaluation.
Performance Improvement Activities- Leadership role within divisional Pressure Injury Prevalence & Incidence Studies with applicable follow up patient assessments, plan of care and action planning. Participation in RCAs and reporting related to hospital acquired pressure injuries. Support/Collaborates on system-wide projects/initiatives related to skin, wound, ostomy, & continence issues.
Position Qualifications Required:
Required Experience: 1 year of wound and ostomy experience desired in acute care setting.
Required Education: BS or BSN degree required, Graduate from approved Wound education program and Graduate or actively enrolled in an approved Ostomy education program and Graduate from accredited RN School of Nursing Program
Training / Certification / Licensure: Current RN License in State of NJ required. BLS required. Driver’s License required.
Wound and Ostomy Board certifications required within 1 year of date of hire
(Current certifications for CWOCN; CWCN/COCN; CWON; or WCC/OMS) or approved National Board Certification for Wound/Ostomy is required or actively pursuing credentialing for one per specialty of the Wound and Ostomy credentialing above)
