Physical Therapist (PT) - Home Health - Glen Carbon, IL

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our partner, a leading home health agency in Glen Carbon, Illinois. In this role, you will provide essential physical therapy services to patients in their homes, helping them regain mobility, function, and independence. 1 1. The Responsibilities :
- Evaluate patients' physical function, including their range of motion, strength, balance, and coordination.
- Develop and implement individualized treatment plans to address patients' specific needs and goals.
- Provide a variety of therapeutic interventions in patients' homes, such as :
Therapeutic exercises.
Manual therapy.Gait training.Functional mobility training.Fall prevention strategies.Patient and family education.Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated care.Maintain accurate patient records and documentation using the Epic electronic health record system.Supervise Physical Therapist Assistants (PTAs) as needed.Adhere to all safety and ethical guidelines.Qualifications :
Doctorate of Physical Therapy (DPT) degree from an accredited program.Current Illinois state license as a Physical Therapist.Minimum of 1 year of experience as a Physical Therapist.Home health experience is preferred.Proficiency in evaluating and treating a variety of medical and surgical conditions in a home setting.Strong understanding of evidence-based practice in physical therapy.Excellent communication, interpersonal, and case management skills.Current BLS certification.Schedule :
Full-time position, Monday - Friday, 8 : 00 AM - 4 : 00 PM.Every other weekend required (per BJC policy).5-6 patient visits per day on average.Benefits :

Primarily geriatric and middle-aged adults.No pediatric patients.Diverse patient population with various diagnoses, including orthopaedic, cardiac, neurological, pulmonary, diabetes, and other conditions.COVID-19 Vaccine : Not required.
Flu Vaccine : Required (medical / religious exemptions may be granted).
Additional Details :
