Responsibilities

• Deliver primary care services to patients of all ages, including wellness exams, preventive care, acute illness management, and chronic disease management.
• Conduct thorough health assessments, order and interpret diagnostic tests, and develop individualized treatment plans.
• Prescribe medications, provide patient education, and manage referrals to specialists as needed.
• Collaborate effectively with the healthcare team to ensure continuity of care and optimal patient outcomes.

Qualifications

• Board Certification as a Family Nurse Practitioner
• Active and unrestricted NYS License
• Excellent clinical assessment and decision-making skills
