Virtus Healthcare Staffing is seeking a dedicated Nurse Practitioner/Primary Care Provider to join our partner medical center in Stockton, CA. Responsibilities
- Patient Assessments: Perform in-person comprehensive assessments, including initial, semi-annual, and unscheduled assessments. Provide continuous medical coverage to participants.
- Care Planning: Collaborate with physicians and the interdisciplinary team (IDT) to develop individualized care plans for each participant.
- Discharge Planning: Work with physicians and the IDT to facilitate smooth transitions for participants discharged from acute or long-term care settings.
- Nursing Home Care: Help maintain and oversee the care of participants residing in nursing homes, conducting regular visits as needed.
- Care Coordination: Collaborate with physicians, community-based providers, and the IDT to ensure effective care coordination.
- Quality Improvement: Review outcomes and utilization data, including hospitalizations and specialist referrals, to identify areas for improvement and contribute to best practices and policy enhancements.
- On-Call Coverage: Participate in a rotating on-call schedule, providing coverage during nights and weekends.
- Education: Graduation from an accredited School of Nursing.
- Certification: Certified as a Nurse Practitioner with Prescriptive Authority.
- Licensure: Active Nurse Practitioner State License.
- Experience: Minimum of two years of experience working with the senior population.
- Knowledge: Current knowledge of chronic care/geriatric issues and best practices.
- Other: Valid State Driver's License with a clean DMV record and proof of vehicle insurance.
