About The Opportunity
The Director of Case Management will play a crucial role in ensuring the delivery of high-quality, cost-effective patient care through the effective management of resources and utilization review.
Responsibilities
- Oversee the daily operations of the Case Management department.
- Supervise, mentor, and evaluate Case Management staff performance.
- Develop, implement, and update Case Management policies and procedures.
- Ensure departmental compliance with all relevant regulatory requirements and accreditation standards.
- Collaborate effectively with interdisciplinary teams (physicians, nurses, social workers, etc.) to facilitate optimal patient care coordination and discharge planning.
- Monitor and analyze utilization data to identify trends, variances, and opportunities for process improvement.
- Educate hospital staff, patients, and families on Case Management processes, resources, and criteria.
- Participate actively in hospital committees and initiatives related to quality improvement, patient flow, and patient safety.
- License: Current California RN license (Required).
- Education: Bachelor of Science in Nursing (BSN) (Required minimum); Master of Science in Nursing (MSN) (Preferred).
- Experience: Minimum 3 years of clinical case management/utilization review experience within a hospital setting, including demonstrated leadership experience (Required).
- Certification: Current Basic Life Support (BLS) certification (Required).
- Skills: Strong leadership, communication, critical thinking, and interpersonal skills (Required).
