What are the responsibilities and job description for the Community-Based Prevention position at VisionQuest?

The Community Based Prevention Specialist (CBPS) will directly assist the family by providing curriculum-based parenting skills education and coaching to improve parenting in relation to expectations of children, parental empathy towards children’s needs, non-violent discipline, parent-child family roles, and children’s power and independence. Must be able to respond in a manner that is culturally responsive to complex/multi-stressed families, must be able to respond to crises. The CBPS works closely with the IFRS Case Manage, Supervisor and LCCYS Case Worker The CBPS is required to maintain a flexible, organized and efficient work schedule and is subject to extended work hours, weekends, and to be on-call as needed.
ESSENTIAL FUNCTIONS:
- Referrals: responds to all referrals in accordance with contractual requirements for timeliness, documentation and communication.
- Provides support and case management functions to address needs and concerns as outlined in referral and service plan; incorporate family’s voice, preferences and needs
- Provides parent education activities using approved curriculum
- Completes training activities and guidance using agency LCCYS approved best practice modalities
- Works with Program Supervisor and Case Manager to ensure that families’ needs are addressed. Assist with the completion of all family assessments and reports as required by the program.
- Carry a caseload of approximately 10 families.
- On-call responsibility to support families in the event of crisis.
- Performs record keeping functions in accordance with program policies.
- Maintains files using agency electronic health record
- Adheres to the policies, procedures and standards as prescribed by LCCYS.
- Maintains confidentiality
- Demonstrates proficiency with electronic health record as appropriate.
- Attend all organizational required training and departmental and program meetings to ensure that up-to-date information is received and/or information of policy changes or practice are adhered to.
- Attend training that will enhance professional growth in the area of case manager and documentation or other topics as deemed appropriate by the program director.
- Travel as needed for training, conferences or to transport youth to destinations located within the U.S.
- Maintain a safe, clean and hazard-free work area.
- Ensure the proper supervision of youth at all times.
- Able to react to change productively and handle other essential tasks as assigned.
- Provide transportation for families if needed
SKILLS & ABILITIES:
- Excellent verbal and written communication skills.
- Organized with attention to detail.
- Ability to work independently and as a member of a team.
- Proficient/skilled in Microsoft Office applications.
- Can perform the duties of the job in a stressful environment.
- Knowledge of basic office equipment use including copy machine, scanner, computer, printer.
- Strong written, verbal, presentation and communication skills
EDUCATION & EXPERIENCE:
- Associate’s degree in Social Work or a related field (preferred)
- At least one year of child-welfare work experience
QUALIFICATIONS:
- Must be 21 years of age or older.
- Flexible schedule (evening, weekends, holidays) to facilitate program services.
- Cleared all background checks from appropriate entities.
- Obtain and maintain First Aid Certification (CPR) and Level 1 fingerprint clearance card.
- Must have a valid driver’s license and be eligible to drive to facilitate program services as required.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS:
- May lift and carry up to 10 pounds frequently, up to 20 pounds occasionally, up to 50 pounds infrequently.
- Performs tasks that may involve standing or walking 50% of the day; sitting 50%of the day.
- May bend, stoop, reach and grasp as necessary.
The pay range for this role is:
21 - 21 USD per hour(480 Community Based Prevention)
