Position: Client Care Manager
Location: Based in Wheat Ridge, with a secondary office in Aurora. Client visits are throughout the city of Denver and North Aurora.

Key Responsibilities:
Client Relationships: Build warm, empathetic connections with clients, their families, and caregivers.
Care Planning: Conduct in-home consultations, onboard new clients, and design personalized care plans.
Team Collaboration: Work with our team to match caregivers with clients and ensure smooth introductions.
Support & Education: Provide ongoing resources and guidance to clients and families.
Networking: Build and maintain relationships with local referral sources (hospitals, assisted living facilities, etc).
