What are the responsibilities and job description for the Field Registered Nurse (RN) Case Manager for Home Care-MERCER COUNTY position at Visiting Angels of Mercer & Burlington Counties, NJ?
Shift Type : Per Diem
Compensation : $70 / visit for initial assessment and $60 / visit for 60-day visits
Visiting Angels is seeking a Home Care Field Registered Nurse (RN) / Case Manager for in-home visits to our clients throughout Mercer County and ideally some Burlington County clients, as well (optional).
This is a non-medical per diem position
There are no set hours for this position
Our Home Care Field Registered Nurses (RNs) prepare thorough and accurate Initial Assessments and Reassessments and develop personalized care plans for our clients. This part-time / per diem position is ideal for an RN who desires a significant level of work but with flexible hours. Most visits to our clients occur on weekdays.
Responsibilities include :
Setting appointments with Visiting Angels' clients in their Mercer County homes during a regular Visiting Angels' care shift; mileage is reimbursed per Company policy
Creating in-home initial client assessments and care plans based on clients' specific care needs
Conducting follow-up in-home assessments at specified intervals
Conducting periodic telephone assessments
Revising care plans as necessary
Supervising Certified Home Health Aides (CHHAs)
Performing Orientations / Skills Assessments of CHHAs
Qualifications include :
Previous Home Care RN experience HIGHLY preferred
Current New Jersey Registered Nurse (RN) license
Ideal candidate resides in Mercer County
Enjoys meeting and talking with older adults
Prior Registered Nurse (RN) experience with in-home care assessments
Excellent written and verbal communicator with strong interpersonal skills
Must be self-motivated, organized, and able to work independently
Able and willing to travel to clients' homes throughout Mercer and / or Burlington Counties
Has a valid driver's license and a reliable car, willing to travel to clients' homes
Is adept with digital / computer skills (data entry on a web-based program, email, scanning, texting)
Has weekday flexibility
