FOCUS & TASKS:
·Take lead calls; conduct telephone & face-to-face interviews for prospective clients.
·Schedule and facilitate initial and on-going client and Home Care Aide meetings.
·Create a care plan with the client needs and update as needed.
·Review services, care cost, complete service agreement & initial paperwork. Meet with client and family to ensure they understand the home care plan and their responsibilities.
·Schedule, conduct, & document home visits to collect feedback regarding Home Care Aide performance. Problem solving as needed to ensure that services are being provided as per the care plan.
·Resolves complaints and incidents.
·Evaluates client and employee satisfaction.
·Assess Home Care Aide performance; provide Human Resources with feedback for Home Care Aide's performance reviews.
·Remind Home Care Aides about policies & procedures in client homes, as needed.
·Acknowledge good job performance & promote Home Care Aide recognition.
·Maintain positive relationships with all clients and referral sources.
·Document thoroughly as directed in a timely manner.
·Maintain absolute confidentiality of all information pertaining to employees, clients and clients’ families.
·Develop & maintain a good working relationship with all staff and management
SPECIFIC JOB KNOWLEDGE, SKILLS AND PERSONAL ABILITIES REQUIRED:
·Associate Degree and two year of supervisory experience in home care is preferred.
·Obtain and maintain registration as a Home Care Aide.
Proficiency with Microsoft Office (Word and Excel) applications, scheduling systems and other health care industry related software.
·Listen & communicate clearly, fluently and diplomatically orally and in writing.
·Ability to remain flexible, resilient and calm in challenging situations.
·Present a well-groomed, professional image.
·Maintain a good driving record.
·Plan, organize, prioritize, delegate and follow through in work activities with time constraints and interruptions to meet deadlines & work independently with a minimum direction.
·Generate goodwill for the Agency with staff, clients, their family members and referral sources. Demonstrate a strong commitment to client service excellence.
PHYSICAL/ENVIRONMENTAL DEMANDS:
·Sitting, standing, bending, reaching, stretching, stooping, walking, climbing stairs and moving intermittently during working hours.
·Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
·Maintain verbal & written communication with co-workers, supervisors, clients, family members, and all business associates within or outside the agency.
