Essential Duties and Responsibilities
- Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident.
- Manage chronic conditions and acute illnesses, providing timely interventions and appropriate referrals as needed.
- Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning.
- Prescribe medications and treatments within the scope of practice and in accordance with state regulations and facility policies.
- Participate in care initiation for our chronic care management services.
- Provide education and counseling to residents and their families regarding health maintenance, disease prevention, and self-care strategies.
- Participate in quality improvement initiatives and interdisciplinary meetings to enhance the overall quality of care and resident outcomes.
- Maintain accurate and up-to-date documentation of all assessments, interventions, and communications in the electronic medical record system.
Requirements
- Master’s degree in Nursing and graduate of an accredited Nurse Practitioner program.
- Unrestricted New York State Nurse Practitioner license.
- Unrestricted Drug Enforcement Administration (DEA) permit to prescribe all medications commonly prescribed in the course of the practice.
- Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid.
Preferred Experience
- A minimum of 3 years of clinical experience, preferably in long-term care.
- Experience with eClinicalWorks (eCW) electronic health records.
