Hospice On-Call Registered Nurse at the VNH focuses on providing pain management and comfort for patients with terminal or chronic illness as well as coordinates and communicates with our interdisciplinary team members and the patient and their families/caregivers.
How you’ll make an impact as a Hospice On-Call Registered Nurse / RN:
- Provide hospice patient care after-hours and weekend, as needs arise for our patients.
- Enable patients to spend quality time with their loved ones and doing the activities they enjoy.
- Build trusting compassionate relationships with your patients and their families.
- Empower caregivers by educating them on care for their loved one.
- Help the patient and family maintain their maximum level of comfort and coping.
- Maintains continuity of care in coordinating with the care team and case manager for patients with ongoing needs.
Minimum Qualifications:
- Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
- Graduate of an accredited school of nursing.
- Hospice experience required.
- Medical/Surgical/Oncology nursing experience preferred.
