Responsibilities of a Long Term Care Case Manager:
- Provides community case management services to patients and families receiving home and community based long term care services.
- Case management services assist patients to remain at home with an array of supportive services for patients, families, and caregivers.
- Assists with timely resolution of patient and family problems by completing assessments including: establishing goals and following through with referral to community services as needed.
- Case management may include issues related to paraprofessional services, legal, financial, spiritual, emotional, and/or interpersonal concerns.
- Provides supervision and evaluation of both client care and personnel provided by agency PCAs and homemakers.
- Works as a liaison with other VNH service providers in advocating for coordinated patient services in accordance with individually authorized State service plans.
- Case Manager acts in accordance with VNH and Choices for Care (CFC) Case Management Certification policies and procedures.
- In depth training and orientation is provided for this home visiting position.
Qualifications of a Long Term Care Case Manager include:
- Bachelor's degree in Human Services, Social Work.
- Two year experience working as a social worker within a community setting preferred.
- Knowledge and/or experience with community long term care resources or programs such as Choices for Care or Choices for Independence desired.
- An active RN or LPN license in both VT and NH desired.
- Ability to communicate clearly, verbally, and in writing.
