This position is a 10hrs - 4 days a week (10am-7pm)
The RN Clinical Liaison serves as a vital connection between patients, families, and healthcare providers, ensuring smooth transitions into home health, hospice, or our inpatient hospice unit. This role includes conducting informational visits to educate patients and families about available services with VNA, Hospice of the Wabash Valley, and The Gibson Family Center while guiding them through care options and facilitating the admission process.
Beyond patient interactions, the RN Clinical Liaison plays a key role in the organization’s marketing and outreach efforts. By fostering relationships with hospitals, physicians, skilled nursing facilities, and other community partners, this professional helps drive referrals and increase awareness of the organization’s services. Their clinical expertise, combined with strong communication and marketing skills, enhances both patient care and business growth.
Key Responsibilities:
- Patient & Family Education: Conduct informational visits to educate patients and families about home health, hospice, and inpatient services, assist with care transitions, and support informed decision-making.
- Referral & Admission Coordination: Collaborate with physicians, discharge planners, and case managers to assess clinical needs, facilitate admissions, and ensure a smooth transition into appropriate care services.
- Marketing & Community Outreach: Build and maintain strong relationships with hospitals, skilled nursing facilities, and other referral sources, participate in outreach activities, and represent the organization at community events.
- Business Growth & Strategy: Support marketing initiatives by identifying referral trends, providing clinical insight to business development teams, and contributing to strategies that enhance service awareness and patient access.
Qualifications:
- Registered Nurse (RN) with active state licensure
- Experience in home health, hospice, or inpatient hospice settings preferred
- Strong understanding of Medicare, Medicaid, and private insurance guidelines
- Excellent communication and interpersonal skills
- Ability to build and maintain professional relationships with referral sources and community partners
- Sales and marketing experience in a healthcare setting is a plus
- Valid driver’s license and ability to travel within the service area
