- Assess and counsel patients/families for smooth hospice transitions.
- Coordinate closely with physicians and healthcare partners.
- Drive quality of care and support community outreach initiatives.
- Aid in discharge planning within RWJ Barnabas Hospitals.
- Licensure: NJ RN license
- Education: BSN required.
- Experience: Hospice and community health nursing preferred.
- Skills: Strong communicator with clinical proficiency and MS Office knowledge.
