VNA Care is currently looking for Per Visit Physical Therapists to conduct home health visits in Lincoln, Concord, and Acton. The ideal candidate should have experience in home health and Oasis E.
Responsibilities
- Conduct home health visits as scheduled, between 8 a.m. to 4:30 p.m. on weekdays and 8 a.m. to 6:30 p.m. on weekends.
- Manage time effectively and ensure accurate Oasis documentation.
Qualifications
- Must hold a valid MA PT license in good standing.
- Previous experience in home health and with Oasis E assessment.
- Willingness to travel within a designated territory.
Additional Information
During the orientation period, new employees will undergo a comprehensive training program with paid orientation. The orientation includes three days of classroom training during the first week, followed by on-the-job training with a preceptor and an educator in the subsequent weeks.
