The Social Worker will assess and address social and emotional problems impacting patient care and recovery.

Key Responsibilities:

Contribute to our mission in this role by:

• Participating in a comprehensive clinical orientation to learn key procedures and EMR systems
• Developing and implementing personalized social work plans of care
• Collaborating with professionals on psychosocial issues and interventions
• Providing patients and families with community resource information and linking them with necessary services
• Ensuring accurate documentation and adherence to agency policies
• Following infection control and safety protocols

Requirements:

Requirements for the ideal candidate include:

• Masters Degree in Social Work with current NJ LSW licensure
• At least one year of supervised clinical experience; community health experience preferred
• Excellent verbal and written communication skills
• Strong interpersonal skills and ability to work with diverse populations
• Spanish speaking language a plus

Working Conditions/Physical Demands: Services are provided in patients' homes or community/facility settings
