Job Description
As a Clinical Social Worker, you will play a vital role in assessing the social and emotional needs of our patients and developing effective plans to address these challenges. Your primary responsibilities will include:
- Taking part in a two-week clinical orientation to learn key procedures, the EMR system, compassionate care, and safety
- Developing and implementing a social work plan of care
- Collaborating with professional staff on psychosocial problems and appropriate interventions
- Providing patients and families with information on community resource options and assisting in linking with needed resources
- Adhering to the organizations policy and procedure on documentation
- Adhering to agency infection control and safety policies
You will work closely with our interdisciplinary team to ensure that our patients receive comprehensive care that addresses their physical, emotional, and social needs.
Required Skills and Qualifications
To be successful in this role, you will need to possess the following skills and qualifications:
- Masters Degree in Social Work required with current NJ LSW
- 1 year of supervised clinical experience; community health experience preferred
- Excellent verbal and written communication skills
- Excellent interpersonal skills
- Ability to work with multiethnic and diverse socioeconomic populations
- Spanish speaking language a plus
