Overview :
The VNACJ Focus on Advancing Colorectal Cancer (CRC) Screening Awareness & Education in Monmouth & Ocean Counties (VNACJ FOCUS) program is designed to increase awareness and education for colorectal cancer (CRC) screening by expanding the reach of the existing VNACJ New Jersey Cancer Education and Early Detection (NJCEED) program, which focuses on colorectal cancer in addition to breast, prostate and cervical cancer. Exact Sciences funding will allow for the addition of a dedicated Bilingual Community Health Worker on staff dedicated to education and workshops specifically geared to Colorectal Cancer. Through a multi-faceted approach that combines education, outreach, and advocacy, the program aims to empower individuals to prioritize their colorectal health, overcome barriers to screening, and make informed decisions about preventive care. Responsible for providing basic lay navigation to eligible cancer / health screening sites on Monmouth and Ocean County. Identifies and reduces barriers to care; provides referrals to necessary resources; delivers patient reminders and follow up; recruits age-eligible uninsured or underinsured individuals, without regular medical checkups, for cancer screening. Fulfills grant requirements as specified by Exact Sciences
Responsibilities :
- Supports organizations mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
- Assesses physical and built environment for neighborhood indicators such as mobility and transportation, community safety, number of places and facilities for screening and treatment and access to the same.
- Prepares / Conducts in person (virtual where appropriate) outreach, educational and screening activities on colorectal cancer in targeted areas.
- Eliminate barriers to Colorectal Cancer and reduce barriers to follow up colonoscopy adherence and empower individuals to prioritize colorectal health.
- Boost impact to bring widespread awareness to Colorectal Cancer, leading to prevention and early detection
- Promote shared decision making, patient choice, and screening options
- Promotion of screening through small media
- Establish a patient referral and tracking process, including follow-up as they navigate the different points in the cancer continuum in order to fulfill monthly and quarterly reports.
- Attend appropriate educational opportunities / training
- Adheres to the organizations policy in regards to absenteeism and appearance.
