We are currently looking for an Home Care Intake Coordinator (LPN) to join our team on-site at major hospital in New Brunswick. This position will be responsible for providing assessment of patients’ home health care needs and coordination of referrals to home care and hospice.
Responsibilities
What You’ll Do:
As a Home Care Intake Coordinator (LPN), you will be responsible for assessing patients' home health care needs and coordinating referrals to home care and hospice services.
Clinical Orientation: Participate in a comprehensive two-week orientation to learn key procedures, safety protocols, and the EMR system.
Assess & Plan Care: Evaluate patients with complex health conditions and develop personalized care plans.
Coordinate Care: Manage referrals to home care services, ensuring seamless continuity of care.
Collaborate for Best Outcomes: Work closely with other VNA clinicians and external providers to ensure optimal patient care.
Patient & Family Education: Provide education and strategies for patients and families to manage care at home.
Documentation: Keep accurate records of changes in patient conditions and updates to care plans.
Qualifications
The ideal candidate will have:
LPN License: Graduate of an NLN-accredited LPN program with current NJ LPN licensure.
Experience: 2 years of home health nursing experience preferred.
Skills: Strong communication, organizational, and decision-making skills.
Client Focus: Ability to think creatively and solve problems while providing patient-centered care.
Physical Demands: Fast-paced healthcare environment with frequent phone, email, and computer use. Ability to travel using a personal vehicle.
