Overview :
Monmouth ACTS Community Health Worker will assist with in person client intakes, connecting clients to community resources, community outreach to residents, connecting with new community resources and service providers.
Responsibilities :
- Supports organizations mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationships with co-workers, patients and members of the community
- Maintains expertise in eligibility, enrollment and program specifications for community resource
- Works with individuals and organizations that provide client services or resources (i.e. transportation services, counselors, language translators, and representatives who assist with financial support);
- Adheres to agency infection control and safety policies
- Adheres to the organizations policy in regards to absenteeism and appearance.
