Per Diem Home Care Intake Coordinator

We are seeking a Per Diem Home Care Intake Coordinator RN to join our team on-site at major hospital in New Brunswick. This position will be responsible for providing assessment of patients’ home health care needs and coordination of referrals to home care and hospice. Responsibilities
Here’s how you’ll move healthcare forward in this role:
- Take part in a two-week clinical orientation to learn key procedures, the EMR system, compassionate care, and safety.
- Assess and evaluate the needs of patients with multiple disease processes and comorbidities and develop an appropriate plan of care
- Provides education and strategies for patients and families
- Coordinates referrals to home care to promote continuity of care
- Partners with other VNA clinicians and external providers to execute best patient outcomes
- Documents all changes in patient’s condition and updates care plans and outcomes
Qualifications
The ideal qualifications for the RN Home Care Intake Coordinator include:
Current RN licensure in New Jersey, with graduation from an accredited RN program.
2 years of home health nursing experience, providing you with a deep understanding of patient care and clinical assessments.
Strong communication skills—both written and verbal—to effectively educate and collaborate with patients, families, and healthcare teams.
Organizational skills and the ability to prioritize in a fast-paced environment.
A client-focused mindset with the ability to make confident decisions and problem-solve in a compassionate and creative way.
Working Conditions/Physical Demand: Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and driver’s license
