As a Registered Nurse Home Health for Somerset County at VNA Health Group, you’ll be an integral part of a team dedicated to providing high-quality, compassionate care to patients who need it most. You’ll make a direct impact on their lives by assessing their health needs, developing personalized care plans, and empowering patients and families with the knowledge they need to live healthier lives. Responsibilities:

• Get the Best Training: Start with a two-week clinical orientation to learn everything you need to succeed – from key procedures to our EMR system, safety protocols, and compassionate care.
• Be the Expert: Assess patients with complex health needs, develop personalized care plans, and ensure the best outcomes.
• Make a Connection: Educate and guide patients and families through their healthcare journey – helping them understand their conditions and the path to better health.
• Collaborate for Success: Work closely with a team of VNA clinicians and external providers to ensure seamless care and optimal results for every patient.
• Document with Purpose: Record and track changes in patient conditions, updating care plans to reflect their progress and needs.

Qualifications:

• Registered Nurse (RN) with a Bachelor’s Degree in Nursing (preferred).
• 1 year of clinical experience as a registered nurse (bonus points if you have experience in homecare!).
• Strong verbal and written communication skills – you’ll be guiding and educating patients and families.
• Proficient with technology – navigating EMR systems and using digital tools to enhance patient care.
• A reliable car and valid driver’s license – you’ll be traveling to patient homes, so your own transportation is key.

• Flexibility to work both indoors and outdoors while using clinical equipment.
• Comfortable using a phone, computer, and other digital tools to stay connected with your team and patients.
• You’ll be making a direct impact through client and customer contact, so your presence is invaluable.
