Key Responsibilities
Your primary responsibilities will include:
- Assessing the social and emotional needs of our patients and developing effective plans to address these challenges
- Collaborating with professional staff on psychosocial problems and appropriate interventions
- Providing patients and families with information on community resource options and assisting in linking with needed resources
- Developing and implementing a social work plan of care
- Ensuring that our patients receive comprehensive care that addresses their physical, emotional, and social needs
You will work closely with our interdisciplinary team to ensure that our patients receive high-quality care that meets their unique needs.
Qualifications
To be successful in this role, you will need to possess the following skills and qualifications:
- Masters Degree in Social Work required with current NJ LSW
- 1 year of supervised clinical experience; community health experience preferred
- Excellent verbal and written communication skills
- Excellent interpersonal skills
- Ability to work with multiethnic and diverse socioeconomic populations
- Spanish speaking language a plus
