We are currently looking to hire a Registered Nurse to join our Hospice team in Essex County on a per diem basis. The person in this role will be responsible for providing skilled nursing care for Pediatric Hospice patients and families.
Responsibilities
1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.2. Performs physical assessment and evaluates the needs of Hospice patients and develops an appropriate plan of care3. Documents all changes in client’s condition, updates care plans and outcomes4. Identifies learning needs and plans appropriate teaching strategies for individuals and families5. Monitors quality and appropriateness of services provided 6. Provides and follows individualized care plan set up by the case manager.7. In communication with the RN case manager and the Hospice Medical Director on any changes or concerns one has after an assessment of the pediatric hospice patient,8. Adheres to agency infection control and safety policies9. Adheres to the organization’s policy in regards to absenteeism and appearance
Qualifications
1. Active New Jersey Registered Nurse License2. 1 year of pediatric Health experience 3. Excellent verbal and written communication skills4. Strong computer skills5. Car and valid driver’s license
Working Conditions/Physical Demand: Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and customer contact.
