ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

- Perform homemaking duties for patient/clients in their home or other designated facility including light housekeeping, light food shopping, errands, pick up medications from pharmacy and meal preparation.
- Report and document patient/client’s needs and responses to the appropriate Case Manager and/or Manager. Report patient/client’s change in condition or situations immediately.
- Adheres to patient/client Homemaker care plan as set up by the primary nurse/therapist while maintaining a healthy, safe environment.
- Provide non-personal care while promoting client/patient dignity and provide a calm, supportive and therapeutic environment for client/patients and their families.
- Provide excellent customer service to patients/clients.
- Utilize supplies and equipment in a cost-effective manner.
- Document adequately to ensure prevention of write-offs.
- Refer questions regarding costs of care or insurance coverage to appropriate Case Manager or Clinical Manager.
- Perform other duties as assigned.
- Adhere to guidelines that prohibit the following tasks from being performed in this role including:
- Heavy seasonal house cleaning such as washing floors, curtains, woodwork;
- The family wash or ironing;
- Dressmaking or tailoring;
- Lending or borrowing money or handling of any family business affairs;
- Administration of medications;
- Give personal address or telephone number
- Cannot transport any clients in their car.
- Perform any duties or procedures outside of this job description
Requirements
EDUCATION AND EXPERIENCE
- High school diploma or equivalent preferred.
- Minimum 6 months experience in a related field preferred.
REQUIRED SKILLS AND ABILITIES
- Demonstrate competency in carrying out non-personal care.
- Ability to travel required. Travel throughout central New Hampshire to observe Home Health care service and other job-related activities is required.
- Ability to function effectively as a member of a team.
- Current drivers’ license required.
- Proof of automobile insurance required.
- Ability to pass a motor vehicle background screening.
- Ability to work in an environment where latex may be present.
- Basic computer skills to complete required documentation.
- Demonstrate a positive demeanor toward staff and clients.
PHYSICAL DEMANDS/WORK ENVIRONMENT
- Job requires some physical activities including but not limited to; sitting or standing for extended periods of time, typing, reaching, standing, walking, kneeling, lifting, fingering, grasping, feeling, talking and hearing.
- Job requires use of general office equipment including phones, copiers and computer equipment.
- Job requires light to medium physical work including the ability to exert up to 25 lbs. of force occasionally, 10 lbs. regularly.
- Job requires visual acuity including the ability to see at or within arm’s reach as well as to operate a motor vehicle.
- Job is subject to inside environmental conditions; low noise level.
