Perform delegated licensed nursing assistant and homemaking duties for patient/clients in their home or other designated facility. Under the general direction of the Director of Home Health and Hospice and the direct supervision of the Registered Nurse and within established policies and procedures. Assist Private Duty Manager as needed with administrative office tasks.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

- Adhere to patient/client LNA care plan as set up the while maintaining a healthy and safe environment.
- Reports and documents patient/client’s needs and responses to the Nursing supervisor.
- Reports patient/client’s change in condition or situations immediately.
- Provide physical support to assist patients to perform daily living activities, such as getting out of bed, bathing, dressing, using the toilet, standing, walking, or exercising.
- Turn or reposition bedridden patients.
- Record vital statistics or other health information.
- Review patients' dietary restrictions, food allergies, and preferences to ensure patient receives appropriate diet.
- Measure and record food and liquid intake or urinary and fecal output, reporting changes to medical or nursing staff.
- Monitor patients to detect health problems and advise Nursing supervisor.
- Perform light housekeeping and Homemaker duties deemed acceptable by the agency in conjunction with or in the absence of the agency Homemaker.
- Promote client dignity during provision of care.
- Provide calm, supportive and therapeutic environment for clients and their families.
- Provide education to staff and facilities regarding client care.
- Fax patient visit schedules and documentation to skilled nursing facilities.
- Prepares reports, in-services, and other documents as requested.
- Assist Private Duty Manager as needed with the following;
- Maintaining Private Duty wait list. Updating this wait list monthly, calling to still determine need and sending letters out to clients with a status update.
- Supervisory visits that are due every six months and as needed or requested by clients. Visit clients in their home, review the supervisory questions, and document correctly in the client chart under Supervisory Visit.
- Yearly redeterminations for clients when their grant program is due to expire. Visit client and fill out appropriate application with them, gather income information, have client sign, date, and return completed application to the Private Duty Manager.
- Filling out and mailing approval letters to clients after state redetermination is complete and received in the mail.
- Make copies of Private Duty paperwork and assemble Private Duty admission folders.
- Private Duty Homemaker admissions. Visit with client to determine needs and qualification, perform a safety check on the home environment, and create a care plan with the client. Enter client into My Unity as a new patient.
- Managing and sending out HHA and Homemaker daily schedules and filling openings or moving cancellations, particularly when Private Duty Manager is out of the office.
- Private Duty Quarterly state reporting excel sheets.
- Homemaker missed visit forms.
- Use effective administrative support skills to complete all assigned tasks and projects.
- Perform LNA visits as needed.
- Perform other duties assigned.
Requirements
EDUCATION AND EXPERIENCE
- High School diploma.
- Current Certification/Licensure as an LNA in the State of New Hampshire is required.
- Minimum of 2 years of progressive relevant professional experience, preferably in a homecare setting.
SKILLS AND ABILITIES
- Ability to express or exchange ideas by means of the spoken word. Primary functions include activities in which incumbent must convey detailed or important spoken instructions to patients, physicians, families and other employees accurately, loudly or quickly
- Ability to receive detailed information through oral communication and to make discriminations in sound.
- Ability to perform job alone following standard practices and any of several procedures but refer questionable cases when they arise out of standard practice.
- Ability to demonstrate courtesy, compassion and respect in relating to patients, families, visitors, physicians and co-workers.
- Ability to work as a team and maintain positive working relationships with employees and healthcare professionals.
- Ability to document patient care accurately according to nursing standards and plan of care.
- Working knowledge of Microsoft Office software and other industry specific programs.
- Must demonstrate ethical conduct and build trust with patients and family members.
- Ability to follow safety guidelines as instructed by the agency.
- Ability to wear protective clothing such as mask, glove, head cover, lead apron, safety glasses, safety shoes, etc.
- Ability to travel required. Travel throughout central New Hampshire is required.
- Current drivers’ license is required.
- Proof of automobile insurance is required.
- Ability to pass a motor vehicle background screening is required.
PHYSICAL DEMANDS/WORK ENVIRONMENT
- Job requires physical activities including but not limited to; standing for extended periods of time, bending, lifting, reaching, kneeling, pushing, pulling, crouching, crawling, fingering, grasping, feeling, talking and hearing.
- Job requires moderate physical work including the ability to exert up to 50 lbs. of force occasionally, 30 lbs. regularly.
- Job requires close visual acuity including the ability to see at or within arm’s reach.
- Job is subject to inside environmental conditions; low noise level.
