Home Health RN Clinical Manager
Location: Hanover, PA

Position Type: Full-Time, Exempt, Monday-Friday, 8:00 am-4:30 pm
Summary:
We are seeking an experienced and dedicated Home Health Clinical Manager to lead our interdisciplinary team, including RNs and LPNs. The Clinical Manager will ensure the delivery of high-quality care while fostering staff development and operational efficiency. This position requires a compassionate leader committed to upholding the organization's mission, vision, and values.
Key Responsibilities:
Patient Care Coordination
- Oversee scheduling and ensure adherence to individualized care plans.
- Conduct team meetings and patient care conferences.
- Collaborate with patients, families, physicians, and care teams for optimal outcomes.
Personnel Management
- Supervise and evaluate clinical staff performance.
- Assist in the recruitment, onboarding, and orientation of new team members.
- Conduct performance evaluations and staff development initiatives.
Operational Oversight
- Manage caseloads and productivity.
- Ensure compliance with regulatory standards.
- Participate in administrative on-call rotations and insurance authorizations.
Leadership
- Promote a collaborative work environment.
- Facilitate professional growth and team cohesion.
- Act as a resource for clinical problem-solving and decision-making.
Performance Improvement
- Work with Quality Improvement to enhance patient outcomes.
- Ensure compliance with Medicare and PA Home Health Licensure standards.
Customer Service
- Foster positive relationships with internal and external stakeholders.
- Maintain accountability for patient and staff satisfaction.
Qualifications:
- Current PA RN license, BSN preferred.
- Minimum 3 years of clinical experience in Home Health.
- Strong leadership skills
- Valid driver’s license with reliable transportation.
- CPR certification.
- Strong leadership, communication, and organizational skills.
