Position Summary:
Join our Home Health team as a Licensed Practical Nurse (LPN), providing skilled nursing care to patients, families, and caregivers under the direction of the RN Manager. You will ensure compliance with Medicare Home Health Conditions of Participation, Pennsylvania licensure laws, and agency policies. Your role will include documenting patient services, collaborating with team members, and delivering compassionate, patient-centered care.
Key Responsibilities
Patient Care:
- Deliver safe, high-quality Home Health services.
- Implement and follow the patient’s plan of care as directed by the RN.
- Administer medications according to Pennsylvania Nurse Practice Act.
- Educate patients, families, and caregivers on diagnoses and care plans.
- Monitor patients’ conditions, report changes, and adjust interventions as needed.
- Assist patients with daily activities and encourage self-sufficiency.
- Maintain a safe environment for patients and support continuity of care.
Documentation & Communication:
- Submit accurate clinical notes on patient conditions and care provided.
- Collaborate with the RN Case Manager or Clinical Manager for additional interventions.
- Participate in performance improvement activities and clinical record reviews.
Resource Management:
- Plan travel and manage caseload efficiently.
- Participate in evening and weekend rotations as required.
- Ensure compliance with all applicable laws, regulations, and agency policies.
Customer Service:
- Deliver care with empathy, respect, and professionalism.
- Respond to questions, concerns, and feedback with courtesy.
- Maintain accountability for patient satisfaction throughout the care process.
Qualifications:
- Graduate of an accredited practical nursing program.
- Licensed Practical Nurse licensed in Pennsylvania.
- At least one year of nursing experience preferred (Home Health, medical-surgical, or critical care).
- Valid Pennsylvania driver’s license and reliable transportation with insurance.
- Current CPR certification.
- Satisfactory criminal background check and professional references.
Physical Requirements and Work Conditions:
- Ability to work in various home and facility settings.
- Must have sufficient hearing, vision, and manual dexterity to perform essential functions.
- Effective stress management and communication skills required.
