Occupational Therapist – Home Health and Hospice
VNA Hanover and Spring Grove
Position Overview:
Join our compassionate team as an Occupational Therapist (OT), providing essential care to home health and hospice patients. You’ll play a pivotal role in helping individuals achieve their optimal health goals through personalized therapy and support, collaborating with a multidisciplinary team.
Key Responsibilities:
- Evaluate patient needs and develop personalized therapy plans.
- Provide training on adaptive/assistive devices and energy conservation.
- Educate patients and caregivers on daily living skills and therapy goals.
- Maintain accurate records in compliance with Medicare and agency policies.
- Participate in performance improvement and team meetings.
Requirements:
- Degree in Occupational Therapy from an accredited institution.
- Current Pennsylvania OT license or in the process of obtaining one.
- Minimum of 2 years clinical experience; home health preferred.
- CPR certification.
- Valid PA driver’s license, reliable transportation, and auto insurance.
Physical Requirements:
- Frequent standing, walking, and occasional lifting (up to 50 lbs).
- Ability to work in varied home environments.
