What You’ll Do:
Provide in-home physical therapy services to patients recovering from illness, injury, or surgery
Develop individualized care plans and collaborate closely with RNs, PTAs, OTs, SLPs, and other team members
Offer palliative therapy to hospice patients when needed, focusing on comfort, mobility, and independence
Supervise Physical Therapist Assistants (PTAs) to ensure quality of care
Educate patients and families on exercises, safety, and therapy goals
Keep accurate clinical notes in line with Medicare and agency standards
Be a key part of care team meetings and performance improvement efforts
Who You Are:
✅ Licensed Physical Therapist in Pennsylvania (or license in progress)
✅ Bachelor’s or Master’s degree in Physical Therapy from an accredited program
✅ At least 2 years of PT experience (home health experience is a plus!)
✅ Strong communication skills, empathy, and a desire to serve
✅ CPR certified
✅ Valid driver’s license, reliable transportation, and current auto insurance
