Position Overview :
As a Home Health Licensed Practical Nurse (LPN), you will play a crucial role in delivering personalized and holistic care to patients within the comfort of their homes. Working as part of a multidisciplinary team, you will assess patient needs, administer medication, implement care plans, and provide support to enhance overall well-being.
Key Responsibilities :
- Patient Assessment :
Conduct thorough patient assessments, including physical and psychosocial evaluations, to develop individualized care plans.
Administer prescribed medications and treatments according to physician orders and established protocols.
Provide skilled nursing care, including wound care, dressing changes, and monitoring of vital signs.
Educate patients and their families on health conditions, treatment plans, and self-care techniques to promote overall wellness.
Collaborate with the interdisciplinary team to develop and update patient care plans based on ongoing assessments and changing needs.
Maintain clear and concise communication with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
Accurately document patient assessments, interventions, and progress in accordance with regulatory standards and agency policies.
Adhere to all regulatory requirements, including maintaining confidentiality and safeguarding patient rights.
Qualifications :
Graduation from an accredited practical nursing program.
Minimum of 2 years of nursing experience, preferably in home health or a related setting.
Strong clinical assessment and critical thinking skills.
Basic Life Support (BLS) certification.
