We are currently looking for a PRN Licensed Practical Nurse (LPN) Intake Coordinator to join our Intake team.
INDNURSES
Responsibilities :
Illustrative duties may include, but are not restricted to, one or more of the following :
- Provides education and information regarding VNA programs, policies and services to the local community, healthcare facilities, physicians, patients and their families.
- Works closely with skilled nursing facilities, physicians offices, the community and VNA staff to facilitate the patient transition to home care.
- Maintains and develops contacts and knowledge of the resources that are available within the local community.
- Ability to use knowledge and skills to convert informational calls into home care referrals.
- Ability and commitment to meet monthly budget numbers.
- Responsible for the entry of patient information accurately and timely as part of the electronic medical records system.
- Answers all incoming calls in a timely manner. Effectively manages calls from external referral sources. Maintains an accurate record of the origin and the response to all incoming calls.
- Identifies unmet learning needs and seeks assistance from others in meeting those needs.
- Complies with company policies and procedures.
- Completes all required forms, reports, time cards, itineraries, etc, accurately and within designated timeframe.
- Recognizes and follows established lines of communication.
- Performs other duties as assigned.
