Job Summary
We are seeking a Patient Care Coordinator who will be responsible for managing daily operations and ensuring smooth coordination of patient care for VNA Home Health. This role serves as the primary point of contact for patients, families, and staff, addressing inquiries, coordinating schedules, and facilitating communication across teams. The coordinator works closely with clinical teams to develop care plans, manage patient appointments, and ensure timely service delivery. Additional duties include assisting with admissions, maintaining office operations, and providing support during crisis situations. The role also includes mentoring new staff, contributing to process improvements, and staying current with industry practices. Adherence to policies, maintaining clinical competency, and supporting agency goals are key components of the role.
- Current Registered Nurse (RN) license in the state of practice.
- Bachelors degree in Nursing (BSN) preferred; Associate degree in Nursing (ADN) accepted.
- Previous experience in home health is required.
- Strong communication skills and the ability to work effectively within a team environment.
- Commitment to maintaining patient confidentiality and adhering to HIPAA standards.
- Ability to handle stressful situations with professionalism and empathy.
