In this role, you will care for patients in our Hospice Home (IPU) on Homestead Road in Fort Wayne, IN.

**Daytime Shifts, Part-Time, with Weekend Rotations,  24 Hours per Week **

POSITION OBJECTIVE:

 To provide safe personal care and support for the terminally ill in the hospice facility under the direction of the Registered Nurse. Assist nursing staff with medication management when needed.

 

QUALIFICATIONS:

 High school diploma or GED required.

Completion of 105 hours CNA Certification program (need a copy of Certificate).

Current/active CNA License

                                                                           AND

Minimum six (6) months (one [1] year preferred) CNA/HHA experience, within the last twenty-four (24) months, in the home health, long term care or hospital setting.

                                                                           OR

RN nursing students who have successfully completed courses with a grade of “C” or better, who currently possess an active CNA License, will be said to have met the above qualifications.

 Must have a current QMA certification to practice in the state of Indiana.

 Must hold current CPR certification or obtain it within the orientation period.

 Must be able to safely perform the essential functions of the job without posing a direct threat to the health and/or safety of self or others in the workplace.

  

ESSENTIAL FUNCTIONS:

 Gives client care per agency policies and procedures, following a written Plan of Care.

 Appropriate and safe techniques in personal hygiene and grooming that include bed bath, sponge bath, tub bath, or shower bath.   Shampoo: sink, tub, or bed.  Nail and skin care. Oral hygiene. Toileting and elimination needs: emptying commode and catheter bag; emptying, cleansing, and reinforcing ostomy bag.  Normal range of motion and positioning, ambulating with use of gait belt. Changing linens, adequate nutrition, fluid intake and output.  

 Assists patients with various rehabilitation activities under the direction of a licensed therapist or RN.

 Takes and records temperature, pulse, respiration, and blood pressure when assigned.                       

Meal preparations. 

Assists patients with feeding and feeds patients unable to feed themselves.

Observe the patient’s condition and report significant changes in status to the appropriate nurse.

Computer documents care per agency policies and procedures.

Manages care of assigned patients in a timely fashion.

Maintains clean, safe, and healthy environment in the patient care area.

Assist with oral medication administration under the direction of the skilled nurse while working in the inpatient hospice unit. 

As a member of the interdisciplinary team, participates in patient care conference as assigned to promote holistic care. 

CNA will maintain good personal hygiene and conduct themselves in a polite and professional manner.

CNA must maintain a therapeutic relationship with patients.  This includes confidentiality of personal matters as well as patients’, coworkers’ and Agency business.  Maintains respect for the patient privacy, and property.

Attends educational opportunities and inservices in accordance with agency guidelines to improve clinical practice.

Must be willing to be cross-trained in other departments within the agency.

Any other duties assigned by the supervisor.

Adheres to all Stillwater Hospice policies.

Adheres to Stillwater Hospice privacy practices as described in the Notice of Privacy Practices.  

 Participates in quality improvement activities and/or other agency committees or task forces when requested.  

The published compensation range for this position reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The base pay may be higher or lower based on demonstrated job-related experience.
